WHAT IS ROBOTIC PROSTATECTOMY?

  • Robotic surgery uses a state-of-the-art minimally-invasive surgical platform (Intuitive Surgical- da VinciTM robotic unit).

THE PROCEDURE
How to prepare for robotic radical prostatectomy?

    After your prostate biopsy and staging tests (PET scan or Bone scan) you will discuss your options with your urologist. To proceed with surgery the general instructions are:

  • Basic blood and urine tests to assess fitness for surgery
  • Cardiac evaluation like ECG and 2D-Echo
  • To stop blood thinning medication for 5-7 days before the surgery
  • Anesthesia check-up and assessment
  • Healthy diet and exercise until the surgery
  • Admission the day prior or day of surgery
Results of robotic prostatectomy

Robotic radical prostatectomy is the gold standard for localized prostate cancer.
It has an excellent success rate and more than 80% of men are cured with surgery alone.
It has much lower complications and quicker recovery than open surgery with no compromise in cancer control.
About 20% of men may need radiation after surgery depending on the follow-up PSA.


What are the complications of prostatectomy?

    Every surgery carries a risk of complications. General complications may include bleeding, infection, blood clots, pneumonia, delay in recovery of intestinal function (ileus), etc. In addition, the delicate location of the prostate can give rise to certain specific complications of radical prostatectomy which include:

  • Urinary Incontinence (Leakage of urine): Most men experience some degree of incontinence which recovers by 3-6months in 90% cases.
  • Erectile dysfunction (ED): Recovery after surgery can take 1-2 years and only 50-60% of men will recover completely. Recovery depends on age, how good erections were before surgery and whether nerve-sparing was possible.
  • Infertility: If you think you might want to preserve your fertility then sperm preservation prior to surgery is very possible and successful.
  • Narrowing of the sutured junction between the bladder and urethra (bladder neck stenosis) – This is very rare (<2%) after robotic surgery.
  • Formation of cysts containing lymph fluid (lymphocele)

Pain is quite mild and easily managed with tablet medications.

When will I be discharged from hospital?

Most men are discharged in 2-3 days after the surgery.


When can I resume normal work?

Work can be resumed after catheter removal and once you are comfortable with managing any incontinence you may have. This is usually 2-3 weeks after surgery.


Will I need additional treatment?

About 20% of men will need additional treatment like radiation depending on the final pathology report and follow-up PSA.
